Financial assistance is discounted healthcare provided by Paris Community Hospital/Family Medical Center.
Qualification is based upon the family’s gross income (before taxes).
Financial Assistance is for patients who do not qualify for other assistance programs, such as Illinois Public Aid. Health services must be medically necessary as determined by the attending physician to be eligible for Financial Assistance.
For each office visit in the Family Medical Center, the first $20 in charges is not eligible for write-off and must be paid at the time of the visit.
Uninsured and underinsured patients residing in our Illinois service area are eligible to apply.
Our service area includes Edgar County and areas outside of Edgar County that have the following zip codes: 61850, 61870, 61876, 61912, 61942, 61943, 62420, 62423, 62441, 62442, and 62474.
If you do not reside in our service area, please contact other hospitals in your area to inquire about their assistance programs.
Veterans: Financial Assistance is only available for services approved by the VA to be provided by PCHFMC.
To receive financial assistance, you must fill out an application and submit it with the required proof to Paris Community Hospital or to one of the Family Medical Centers in Paris, Chrisman, or Oakland. Your application cannot be processed until all documentation is received.

No. If you apply for financial assistance through Paris Community Hospital/Family Medical Center, you will be screened to see if you qualify for other assistance programs, such as Illinois Public Aid. If you do qualify for another assistance program, the Financial Assistance Coordinator can help you fill out the proper forms.
The Financial Assistance Coordinator uses an income table (see below) to see if you qualify for financial assistance with Paris Community Hospital/Family Medical Center.
To use the table, simply find your family’s size (two parents and two children are a family of four), and then your gross yearly family income (the total amount of money you earn per year before taxes). The amount of money listed in the table is the most that you can earn to receive that discount.
For example, if you are applying for assistance for just yourself and earn $16,335 per year, you may be eligible to receive a 100% discount for your healthcare services at PCH/FMC. If you earn $19,000 per year, you may qualify for only an 80% discount.
Paris Community Hospital does have the right to change or take away approval if we receive information that shows a change in the family’s financial situation or if the family moves out of our service area.
Example 1: Family of 4 with an income level of $30,000 qualifies for 100% discount.
Example 2: Family of 2 with an income level of $29,500 qualifies for 60% discount.
