We are happy to offer our patients access to their health information through the online patient portal. Whether you are looking to schedule an appointment, request your medical records or view test results, you can have access wherever you are and whenever you need.

Welcome to Your Patient Portal!

Within the portal, you will be able to:

  • View and update your medical history, allergy, and medication lists
  • Send secure messages to your provider
  • Request an appointment/View scheduled appointments
  • View test and lab results
  • View information related to your hospital/inpatient stay

Patient Portal Registration

Not registered? No problem! Just click the "Log in" link. Once directed to the Portal, you will be given the option to create a new account. The patient portal provides information about medical history, contact information, payment history and more. Any information that isn’t available online can be available upon request from the medical staff.

Note that you must be an established patient at PCH/FMC to create a Patient Portal account.

What information is available on the Patient Portal?

Health information at the Family Medical Centers (as of Jan. 12, 2016), as well as inpatient/hospital information (as of April 11, 2017), is available on the Patient Portal.

To request medical record information outside of the Patient Portal:

  • Family Medical Centers (Paris, Chrisman, Oakland), call 217-465-2606, Ext. 4514.
  • Paris Community Hospital, call 217-465-2606, Ext. 4209.
